CONTEST PERIODS

The contest will take place over 7 periods running from January 1st, 2023, at 00h00 Eastern Standard Time (EST) to December 31st, 2023, at 23h59 EST as follows:

CONTEST ELIGIBILITY AND PARTICIPATION TERMS

To participate, the Contestant must during the Contest Period:

• Have shopped in one of the participating Familiprix branches and possess a copy of the invoice.
• Have responded to the survey on the satisfaction of their shopping experience, electronically at the following address: opinion.familiprix.com.
• Not be one of the following individuals:
• i) Familiprix inc. employee,ii) an affiliate member (pharmacy) of Familiprix,
  iii) a Familiprix retailer,
  iv) a Familiprix representative,
  v) one of Familiprix’s promotional or advertising agencies,
  vi) an employee of one of the persons or companies listed in ii), iii), iv), v), or
  vii) any person living in the same household as one of the persons or companies listed in i), ii), iii), iv), v) (including father/mother, brother/sister, including brither-in-law/sister-in-law, half-brother/half-sister, son/daughter, spouse/partner).
• Be a resident of the province of Quebec or the province of New Brunswick.
• Have reached the legal age of majority in their province of residence by January 1, 2024.
• One entry per survey answered.

PRIZES

There will be seven (7) draws based on seven (7) specific periods.

Throughout the Contest Period, a total of seven (7) prizes will be awarded, representing a total value of three thousand five hundred dollars ($3,500) distributed in the form of Familiplus points (total of 350,000 Familiplus points) for winners who are members of the Familiplus program or in the form of $500 Familiprix gift cards (total of $3,500) for winners who are not members of the Familiplus program.

For each specific Contest Period defined below, a prize is offered to the winner as follows:

• If the winner is a member of the Familiplus program, the winner will receive, as the prize, a set of 50,000 Familiplus points, with a monetary value of $500.
• If the winner is not a member of the Familiplus program, the winner will receive, as the prize, a Familiprix gift card with a monetary value of $500.

DRAWING TERMS AND WINNER DETERMINATION

The potential winners, for each of the specified periods, will be selected from all eligible electronic participation forms received during the Contest Period defined above by an electronic random draw. These random draws will take place on the aforementioned dates at approximately 2:00 PM (EST) at the offices of LANLA (hereinafter referred to as the "Administrator"), located in Montreal (Quebec). The draw will be conducted by the Administrator (the contest judge), whose decisions are final and binding on all Participants.

The odds of being selected for the prize depend on the number of entries received during the Contest Period.

Immediately after the contest drawing, Familiprix (Contest Sponsor) will contact the potential winner by email at the email address used to participate in the contest, which must still be valid, or by phone. The potential winner will have a period of SEVEN (7) business days after being contacted by email or phone (the "Contact Period") to communicate with the Contest Sponsor by email or phone.

For the potential winner to be declared the winner, they must comply with all the requirements set forth in the Official Rules and Regulations of the contest, including:

• Having complied with all the terms of participation in the Contest;
• Provide their full name, contact details, and valid proof of identity.
• Having completed and signed the Declaration and Liability Waiver Form within fourteen (14) days of sending it.
• Submit to and answer correctly the following mathematical skill-testing question: 6+14 / (12-7) = _____, which is replicated in the "Declaration and Liability Waiver Form".

In the event that the potential winner does not respond to the Contest Sponsor by email or phone by the end of the Contact Period, or if the potential winner does not comply with the contest requirements, this potential winner will be deemed to have forfeited their rights to claim their prize, and the Contest Sponsor may, at its discretion, cancel the prize or conduct a new draw from the remaining valid entry forms until another potential winner (hereinafter the "Alternative Potential Winner") eligible to be declared the prize winner is selected. The Alternative Potential Winner will have a new Contact Period of SEVEN (7) business days following the date they were contacted by the Contest Sponsor to respond to the Contest Sponsor by email or phone and validate all the necessary conditions to be declared the prize winner. If no Alternative Potential Winner is declared a winner by the Contest Sponsor within forty-five (45) days of the original draw date, the prize will be cancelled.

Before being declared a winner, any selected entry form must be verified by the Contest Sponsor. The Contest Sponsor and the Contest Administrator reserve the right to request proof of age as well as proof that the winner has complied with the Official Rules and Regulations of the contest before awarding the prize. The winner agrees to accept the prize as awarded, and the prize is neither transferable nor exchangeable for a sum of money or anything else.

All decisions made by the Administrator or the Contest Sponsor concerning this contest will be final and binding on all participants.

Once all the above conditions are met, the prize will be deposited into the Familiplus account of the winner, if they are a member of the Familiplus program, or shipped to the winner by mail to the address provided by the winner, by the Contest Sponsor, within fifteen (15) days following the written confirmation from the Contest Administrator that the potential winner has won. The time to receive the prize by mail may vary slightly depending on the delivery times of the carrier responsible for the delivery.

GENERAL RULES

1. All prize claims are subject to verification by the Contest Administrator or its designated representatives.
2. The Contest Sponsor is the sole and definitive reference source for validating prize claims.
3. All surveys and contest entries become the property of Familiprix. Neither Familiprix, its retailers, affiliated members, nor any organization involved in the contest organization can be held responsible for any contest-related documentation that is unreadable, incomplete, lost, stolen, misrouted, not received, destroyed, or delayed for any reason.
4. Familiprix and the Contest Administrator cannot be held responsible for any defective computer parts, software or links, loss or lack of communication ability, or any computer or network transmission that is defective, incomplete, incomprehensible, or erased, which makes a Contest Participant ineligible.
5. Familiprix and the Contest Administrator cannot be held responsible for any direct or indirect damage caused, in whole or in part, by downloading any software or form or by transmitting any information relating to contest participation.
6. Familiprix and the Contest Administrator are not responsible for incorrect or inaccurate information on the participant's registration form, whether caused by website users or by equipment and/or programs associated with or used by Familiprix or the Contest Administrator, or by any technical or human error that may occur during the course of the contest, including during the processing of registration forms.
7. By participating in the Contest, each Participant agrees and releases Familiprix and the Contest Administrator from any liability or loss arising in any way from the prize.
8. Subject to the approval of the Régie des alcools, des courses et des jeux du Québec for Quebec residents, Familiprix and the Contest Administrator reserve the right to cancel, terminate, or interrupt this contest at their sole discretion.
9. The Organizer reserves the right to modify or revise any rule of the official contest rules at any time.
10. By accepting a prize, the winner(s) consent to Familiprix and the Contest Administrator displaying, using, and publicizing his/her/their name(s) and photo(s) in relation to the contest without compensation.
11. The contest is subject to all federal, provincial, and municipal laws and regulations.
12. For residents of Quebec, any dispute regarding the organization or conduct of a promotional contest may be submitted to the Régie des alcools, des courses et des jeux for resolution.
13. Each prize winner agrees to release Familiprix and the Contest Administrator from any liability related to the prize.
14. All expenses and fees not specifically indicated in the official contest rules as being included in the prize are the winner's responsibility.
15. In case of a discrepancy between these official rules and any other contest-related material, these official rules will prevail.
