Printing your blood pressure readings
Your pharmacist or nurse can give you a form displaying your blood pressure results, including a graph showing any changes in your pressure. This can be a good tool to use if you are working towards treatment targets. You can also give it to your family doctor to monitor your medication.
You must already have received a prescription for this condition within the past 2 years. If the prescription comes from another pharmacy, make sure you have proof of your prescription (ex.: the prescription label).
