Anticoagulant therapy - Initial meeting for INR monitoring

Taking warfarin (Coumadin), a blood thinning medication, requires very close monitoring and special education. Your pharmacist will give you the information necessary to properly use this medication and the lifestyle habits to have on a daily basis. After having received your targets from your doctor, the pharmacist will make sure to closely monitor your INR management. This will help you remain within the safe targets.
You must already have received a prescription for this condition within the past 2 years. If the prescription comes from another pharmacy, make sure you have proof of your prescription (ex.: the prescription label).
